Tetralogy of Fallot, 12 weeks

Case report

The following images show a case of tetralogy of Fallot diagnosed in 12 weeks of pregnancy.

Images 1, 2: 12 weeks of pregnancy; the images show a gray scale four-chamber view of the fetal heart, which was not very contributive to the diagnosis of the tetralogy of Fallot in this case (the image 2 represents a fusion of the image 1 with a drawing elucidating the visible structures).

Images 3, 4: 12 weeks of pregnancy; the images show a color Doppler four-chamber view of the fetal heart, depicting the ventricular septal defect as a component of the tetralogy of Fallot (the image 4 represents a fusion of the image 3 with a drawing elucidating the visible structures).

Images 5, 6, 7, and video 1: 12 weeks of pregnancy; the images show a color Doppler four-chamber view of the fetal heart obtained by a 3D color Doppler glass body mode scanning, depicting aortic flow (very well seen red flow) and absent pulmonary flow, due to the pulmonary stenosis making another component of the tetralogy of Fallot (the images 6 and 7 represent a fusion of the image 5 with a drawing elucidating the visible structures; the pulmonary flow was not visible - red arrow with yellow cross on the image 7). The video shows the structures described above in motion (the speed of the video loop was decreased to 25%).

Images 8, 9:  12 weeks of pregnancy; the images show a 3D color Doppler scan of the fetal heart with a massive aortic flow of the overriding aorta (red flow, arrow on the image 9) going up between the chambers (blue flow).

Images 10, 11: 12 weeks of pregnancy; the images show a 3D color Doppler scan of the fetal heart with a flow across the ventricular septal defect (blue flow, arrow on the image 11).

Images 12, 13: 12 weeks of pregnancy; the images show the left ventricular outflow tract of the heart - the aorta (number 3 on the image 13) overrides the interventricular septum (number 2 on the image 13). The image 13 represents a fusion of the image 12 with a drawing elucidating the visible structures; (1 - ventricles of the heart, 2 - interventricular septum, 3 overriding aorta).
