Right aortic arch

Case report

This is a case of a healthy G2 P1 with a non-contributive family and personal history. Patient was referred to our department at 23 weeks of gestation. We diagnosed an isolated Right aortic arch with left ductus arteriosus. The aorta was on the right of the trachea and pulmonary trunk and ductus arteriosus were on the left of the trachea. these vessels formed a vascular ring around the trachea, so called "U-sign".   

There were no other associated findings detected on the ultrasound examination. Patient wished for the pregnancy termination based on the ultrasound findings.

Images 3-6: Views of the upper mediastinum, images do not show a typical 3-vessel view but so called "U-sign" which is a vascular ring around the trachea formed by the pulmonary trunk and ductus arteriosus on the left and aorta on the right.

Image 7: "U-sign", the pulmonary trunk and ductus arteriosus are left to the trachea, the aortic arch is right to the trachea. These vessels form a vascular ring around the trachea and esophagus. The finding of a "U-sign" is typical for the right sided aortic arch.

Videos 1,2: Video 1 shows a 4-chamber view, aorta descending on the right. Video 2 shows a color Doppler demonstrating a vascular ring around the trachea formed by ductal arch and the aortic arch.
